Work Environment

The position is primarily onsite, allowing you to engage directly with patients in their home environment. This setting fosters a unique relationship between you and your patients, as you work together to set and achieve rehabilitation goals in a comfortable, familiar space. You will work Monday to Friday from 9 AM to 5 PM, collaborating with a skilled interdisciplinary team of healthcare professionals.

Key Responsibilities

• Administer direct patient care in compliance with the physician's prescription, ensuring the highest standard of treatment.
• Utilize a variety of evaluation techniques, adapting your approach to the specific needs of each patient.
• Design individualized treatment programs based on therapy goals, focusing on patient progress and outcomes.
• Monitor patient tolerance and progress during treatment sessions, adjusting programs as needed to maximize recovery.
• Document patient progress accurately and promptly in accordance with departmental policies.
• Participate actively in quality improvement initiatives, contributing to program enhancements and patient care protocols.
• Engage in family meetings and collaborate with multidisciplinary teams to ensure holistic patient management.
• Adhere to emergency procedures to guarantee patient safety at all times.

Patient Population / Client Focus

Our patient population varies widely, including individuals recovering from surgery, those with chronic pain, and patients requiring rehabilitation from injuries. Qualifications

• A Bachelor's Degree in Physical Therapy from an accredited program.
• Active physical therapy licensure in California, with a clean record.
• A minimum of two years of clinical experience, particularly in home health or outpatient settings.
• A positive attitude and strong communication skills to foster relationships with patients and their families.
• Flexibility and adaptability to plan and execute therapy in diverse environments.

Compensation and Benefits
