Key Responsibilities

• Manage the complete therapeutic care process for an assigned patient caseload, coordinating efforts with physicians and the interdisciplinary care team.
• Conduct comprehensive patient assessments, developing and implementing individualized treatment plans that prioritize patient engagement and goal achievement.
• Deliver direct physical therapy interventions to patients, using evidence-based techniques that demonstrate measurable improvements in condition and function.
• Utilize assessment findings to evaluate therapeutic progress, modifying care plans as needed to align with patient needs and expectations.
• Provide skilled, purposeful therapy in both clinical and home settings, ensuring treatment aligns with the physician's established plan of care.
• Educate patients, families, and caregivers on therapeutic exercises, adaptive devices, and at-home therapy strategies to enhance recovery and maintenance of health.
• Prioritize and manage your caseload effectively, determining daily staffing requirements and facilitating seamless hand-offs between care providers.

Qualifications
