Key Responsibilities

• Conduct comprehensive evaluations of patients in their home environment, identifying physical impairments or limitations resulting from injury, disease, or surgery.
• Develop and implement personalized treatment plans that include a variety of therapeutic techniques such as exercise regimens, hands-on therapy, and the use of assistive equipment.
• Monitor and document patient progress regularly, adjusting treatment plans as needed to meet established therapeutic goals.
• Educate patients and their families about the necessary therapeutic exercises, movements, and care procedures to boost recovery.
• Provide training to caregivers and other healthcare team members on physical therapy techniques and assistive device usage.
• Stay current with best practices in physical therapy by participating in professional development through workshops and networking opportunities.

Patient Population / Client Focus

Your work will entail interacting with a diverse patient population that includes children, adults, and elderly individuals recovering from various conditions. You will employ a holistic approach to care, considering each patient's lifestyle, home environment, and specific rehabilitation needs. As a home care Physical Therapist, you will have the opportunity to build meaningful relationships with your patients, enhancing their recovery and engagement in their health journey.

Qualifications

• A Bachelor's Degree in Physical Therapy is required; a Doctorate degree in Physical Therapy (DPT) is preferred.
• Current state licensure in Physical Therapy.
• Strong verbal and written communication skills to effectively convey information to patients and families.
• Familiarity with electronic medical records (EMRs) and other health management software.
• Must be flexible, agile, and adaptable to the varying needs of patients in their own home settings.

Preferred Experience

• Prior experience in home care or outpatient physical therapy is beneficial but not required.
