Work Environment

This position is onsite, offering therapy services in patient homes across Lehigh Acres and surrounding areas. Key Responsibilities

• Direct physical therapy treatment focusing on preventing disabilities, alleviating pain, and restoring muscle function in alignment with the personalized plan of care.
• Evaluate and assess the client's level of function and progress by applying a variety of diagnostic and prognostic tests to inform treatment strategies.
• Supervise and guide physical therapy assistants to ensure efficient and proper execution of care plans, nurturing their professional development.
• Conduct home assessments to adapt the rehabilitation environment, offering recommendations that promote patient independence and safety.
• Administer treatment techniques such as therapeutic exercises, traction, and massage, along with the use of physical agents as needed.
• Incorporate the use of assistive devices like prosthetics and orthotics, ensuring correct fitting and recommending modifications to physicians when necessary.
• Maintain accurate clinical documentation, including evaluations, progress notes, and discharge summaries, ensuring compliance with healthcare regulations.
• Engage in interdisciplinary team conferences, helping to shape comprehensive treatment strategies that consider the holistic needs of the patient.
• Provide education and guidance on care plans to clients and their families, ensuring understanding and adherence to home exercise routines.
• Complete timely documentation and case reporting as per branch guidelines, ensuring continuity of care and communication with all team members.
• Monitor changes in patient conditions and suggest modifications to treatment plans as needed, liaising closely with physicians and case managers.

Patient Population / Client Focus

This role primarily focuses on patients who require orthopedic rehabilitation. You will work with individuals recovering from surgeries, injuries, and various orthopedic conditions. The diverse population comprises both elderly patients needing gentle restorative therapies and active individuals seeking to return to high levels of performance. Your ability to assess and adapt evidenced-based practices to meet each patient's unique needs is paramount in fostering their recovery journey.

Qualifications

• A Bachelor's Degree in Physical Therapy from an accredited institution is required.
• A current state licensure as a Physical Therapist in Florida, in good standing.
• CPR Certification and a valid driver's license, along with automobile insurance, are necessary.
• A creative thinker with the ability to make independent decisions while being open to collaboration.
• A minimum of one year of experience in an acute care or home care setting is preferred.

Preferred Experience

• Experience working with orthopedic patients is strongly preferred, along with familiarity with home care settings.
• Proven skills in using a variety of therapeutic modalities and rehabilitation technologies.
• Experience with electronic health record systems and completion of OASIS documentation is a plus.

Compensation and Benefits

We offer a competitive salary ranging from USD 100,000 to 120,000 annually, commensurate with experience and skills.
