Work Environment

The Physical Therapist will operate predominantly in patients' homes, providing on-site assessments and interventions tailored to each individual's situation. This dynamic environment necessitates adaptability and independent decision-making, allowing you to leverage your clinical judgments directly impacting patient outcomes. You will work closely with a supportive interdisciplinary team, including physicians, nurses, and occupational therapists, to ensure that care plans align with the larger continuum of health services.

Key Responsibilities

• Conduct comprehensive evaluations of new patients, gathering crucial data from patients, family members, and interdisciplinary team members to inform treatment plans.
• Develop and implement tailored treatment plans focused on improving mobility and function, aligning with specific patient goals.
• Utilize professional equipment safely and effectively, demonstrating its purpose to patients while adhering to safety protocols.
• Continuously re-evaluate and adjust treatment plans based on patient progress, utilizing evidence-based practice to drive interventions.
• Ensure meticulous documentation of treatment sessions, progress notes, and billing records to maintain compliance with healthcare regulations.
• Provide education to patients and their families about rehabilitation processes, home exercises, and community resources that support recovery and wellness.
• Adhere strictly to the State Practice Act and internal policies, ensuring that all patient interactions prioritize ethical care.

Patient Population / Client Focus

This role primarily supports patients recovering from orthopedic injuries, surgeries, and chronic conditions affecting mobility. Your approach will cater to a diverse age range, ensuring you address the unique developmental and therapeutic needs of older adults, adults, and adolescents. Your ability to recognize and adapt to individual patient needs will be paramount in fostering successful rehabilitation outcomes.

Qualifications

Candidates must possess a Bachelor's degree from an accredited program in physical therapy. Essential licensure as a Physical Therapist in the State of Florida is required, and a temporary permit will be accepted for qualified applicants. Additionally, you should maintain current Basic Life Support (BLS) certification for healthcare providers.

Preferred Experience

While not required, previous experience working in home health settings or rehabilitative therapies with orthopedic patients is highly valued. Familiarity with electronic health records (EHR) systems will be an advantage in ensuring efficient documentation and communication with the care team.

Compensation and Benefits

The position offers a competitive salary ranging from USD 120,000 to 130,000 per year, commensurate with experience and qualifications. Additionally, we provide:
