Key Responsibilities

• Conduct thorough initial evaluations and ongoing assessments to determine resident mobility, pain levels, strength, and overall functionality.
• Develop and implement individualized treatment plans that align with patients' rehabilitation goals and physician recommendations.
• Deliver evidence-based therapy sessions customized for each resident, focusing on improving mobility, managing pain, and preventing further injury.
• Educate residents, their families, and caregivers on therapeutic techniques, at-home exercises, and the importance of continued practice for optimal recovery.
• Collaborate closely with nursing, occupational therapy, and wellness staff to create comprehensive interdisciplinary care plans.
• Maintain transparent and accurate documentation in accordance with Medicare and state regulatory standards, ensuring quality and compliance.
• Contribute to fostering a positive, patient-centered environment that respects the dignity and individuality of all seniors in our care.

Patient Population / Client Focus

The primary focus of this role is to serve the elderly population residing in our rehabilitation facility. This includes working with residents recovering from surgeries, managing chronic pain conditions, and those requiring rehabilitation following hospitalization. Your expertise in geriatrics and understanding of age-related challenges will be invaluable as you help these individuals regain mobility and foster independence, significantly improving their quality of life.

Qualifications

• Current and valid Physical Therapist license in Virginia or eligibility for licensure.
• Bachelor's or Doctoral degree from an accredited Physical Therapy program.
• CPR/BLS certification or the willingness to obtain it upon hire.
• Strong communication and interpersonal skills to effectively engage with residents, families, and the healthcare team.
• Proficient organizational skills to manage patient caseloads effectively and prioritize tasks.
• Ability to work autonomously as well as collaboratively within a care team.

Preferred Experience

• Minimum of two years of experience as a Physical Therapist, preferably in geriatric care or long-term rehabilitation settings.
• Proficiency with rehabilitation tools and technologies that enhance therapy effectiveness.
• Experience with Medicare regulations and documentation processes.

Compensation and Benefits

This full-time position offers a competitive salary ranging from $90,000 to $100,000 annually, complemented by a comprehensive benefits package that includes:
