Key Responsibilities\n

As an LPC at Rethink Mental Healthcare, your day-to-day tasks will include:

• Maintaining open and effective communication with supervisors, direct care staff, families, and other stakeholders regarding patient progress and treatment plans.

• Creating and sharing a tentative schedule with supervisors for client appointments, home visits, and progress monitoring activities.

• Coordinating and facilitating home visits to gather data through observations, interviews with family members, and reviews of relevant documentation.

• Ensuring that the implemented program plans are effectively executed and any necessary adjustments are identified and made promptly.
\n
• Participating actively in interdisciplinary team meetings to ensure a holistic approach to patient care.
\n
• Documenting comprehensive progress notes on a monthly and quarterly basis to track treatment effectiveness.
\n
• Organizing training sessions for staff on program implementation best practices and documentation techniques to ensure compliance with organizational standards.

• Providing follow-up support through interviews, home visits, and observations to adapt care to patient needs as they evolve.

• Maintaining up-to-date records to facilitate easy access to patient information for authorized personnel.

• Serving as a liaison to the broader community and mental health sector to promote awareness and support development goals.

• Assisting in the formulation of training materials to enhance program efficacy and patient outcomes.

Patient Population / Client Focus\n

Qualifications\n

The ideal candidate will possess:

• A Master's or Doctoral degree in Counseling, Psychology, or a closely related field.

• An active and unrestricted LPC license in the state of New Jersey.

• At least two years of counseling experience, preferably within a clinical context.
