Key Responsibilities\n

• Conduct comprehensive mental health assessments that consider the impact of physical health on mental wellness, utilizing standardized assessment tools and methodologies.

• Design and implement individualized treatment plans that address both psychiatric and physical health issues in collaboration with patients and their healthcare providers.

• Facilitate individual and group therapy sessions, employing evidence-based practices tailored to meet the specific needs of each patient, focusing on fostering resilience and personal growth.

• Work in tandem with physicians and other healthcare professionals to coordinate patient care and improve treatment outcomes through regular interdisciplinary meetings.

• Provide crisis intervention services and support to patients experiencing acute mental health episodes, ensuring their safety and immediate needs are met.

• Educate patients about the interconnectedness of mental health and physical well-being, empowering them with strategies to manage their conditions effectively.

• Maintain meticulous and confidential patient documentation that adheres to ethical and legal standards, utilizing electronic health record (EHR) systems to track patient progress and treatment plans.

• Participate actively in team discussions and case conferences to share insights and strategies, build camaraderie, and enhance patient care.

Patient Population / Client Focus\n

Our ideal candidate will work with a diverse patient population, including individuals who experience mental health disorders in conjunction with complex medical conditions. The therapist will be instrumental in guiding individuals through everyday challenges, crises, and life transitions while supporting their mental and emotional health.

Qualifications\n

• A Master's or Doctoral degree in Social Work, Marriage and Family Therapy, Mental Health Counseling, or Psychology from an accredited institution.

• Current and valid licensure as a Mental Health Counselor (LMHC), Licensed Clinical Social Worker (LCSW), LCSW-R, Licensed Psychoanalyst, Psychologist, or Licensed Marriage and Family Therapist (LMFT) in New York State.

• Demonstrable experience working with patients who have co-occurring medical conditions and mental health needs.

• Exceptional interpersonal and communication skills, with a focus on building trusting relationships with patients and collaborating effectively with healthcare teams.

• Professional integrity with the capacity to respect confidentiality and uphold ethical guidelines in all situations.

• Proficiency in utilizing electronic health record systems and standard office productivity software.

• Strong team-player mentality with an eagerness to engage in collaborative care efforts.

Compensation and Benefits\n

The compensation for this position ranges from USD 80,000 to 100,000 annually, commensurate with experience and qualifications.
