Key Responsibilities\n

• Conduct comprehensive psychosocial assessments of patients and their families to identify emotional, social, and environmental strengths, as well as challenges related to their diagnosis and life situation.

• Develop, implement, and evaluate personalized treatment plans using appropriate social work interventions that address individual patient needs.

• Collaborate with an interdisciplinary healthcare team, actively involving patients and families in the planning and implementation of care strategies.

• Facilitate discharge planning by coordinating with consulting physicians, third-party payers, and other healthcare providers, ensuring a smooth transition from hospitalization to home or alternative care settings.

• Deliver education and counseling to patients and families regarding adjustment to illness, enhancing their coping strategies and support systems.

• Utilize crisis intervention techniques and provide both brief and long-term therapies tailored to individual, group, and family needs.

• Serve as a clinical resource to other staff, sharing expertise and providing support as necessary.

• Engage in ongoing clinical evaluation of oncology patients regarding mental health concerns, including anxiety and depression, and provide short-term counseling as needed.

• Participate in a shared on-call rotation to facilitate after-hours support for urgent patient needs.

• Present at educational conferences and contribute to community outreach programs focused on psychosocial care for cancer survivors.

• Support the training and evaluation of new clinical social workers and actively participate in the professional development of students and staff.

You will primarily work with patients undergoing detoxification and treatment for substance use disorders, alongside individuals facing complex emotional and mental health challenges such as depression, anxiety, and trauma. Qualifications\n

• A Bachelor’s Degree in Social Work (BSW) is required, with a current LCSW license in the state of Illinois.

Compensation and Benefits\n

The salary for this position ranges from USD 70,000 to 100,000 annually, commensurate with experience and qualifications.
