Key Responsibilities\n

• Establish rapport and communicate effectively with patients, their families, and the healthcare team using active listening and clear, non-judgmental language.

• Conduct brief, evidence-based screenings and assessments for mental health conditions such as depression, anxiety, and substance abuse, facilitating referrals for further evaluation and treatment as needed.

• Create and manage a patient caseload, providing ongoing support and brief interventions to promote mental well-being.
\n
• Utilize electronic medical records to document patient encounters and treatment plans within 48 hours of each appointment, ensuring efficient healthcare delivery.
\n
• Develop integrated care plans that connect patients with necessary services and facilitate communication among all stakeholders involved in the patient's care.
\n
• Safeguard patient privacy and confidentiality in all communications and documentation.
\n
• Participate in individual and group supervision, case conferences, and quality improvement meetings to enhance team collaboration and care delivery.

• Facilitate referrals to the Center's consulting psychiatrist for clients requiring psychiatric medication management.
\n
• Provide timely responses to inquiries from patients and their families regarding appointments and care concerns.

• Identify and connect patients to community resources that align with their goals and interests, including social services and support groups.

Patient Population / Client Focus\n

The focus of this position is on providing care to a diverse patient population, including individuals facing chronic health conditions and those in need of end-of-life support. Qualifications\n

• Active LCSW licensure in Pennsylvania is required.
