Work Environment\n

This position is remote, allowing for a flexible work environment that adapts to the needs of clients and families. Your typical work schedule will be from Monday to Friday, 8:00 AM to 4:30 PM, but occasional evening or weekend hours may be needed based on client requirements. Flexibility and on-call availability are essential, particularly during urgent situations, to ensure that our seniors receive continuous, high-quality care.

Key Responsibilities\n

• Conduct comprehensive assessments of seniors’ psychosocial needs, considering emotional, mental, and social factors that affect their quality of life.

• Develop personalized care plans in collaboration with clients and their families to address individual goals and preferences.

• Provide counseling and emotional support to clients and families, assisting them in navigating life transitions, health challenges, and personal relationships.

• Advocate for clients' rights, ensuring their access to appropriate services and community resources, from financial assistance to social support networks.

• Collaborate with an interdisciplinary team of healthcare professionals, including nurses and caregivers, to coordinate care and share insights that enhance client outcomes.

• Regularly visit clients to monitor their psychological and physical health, evaluate living conditions, and address potential risks or concerns.

• Facilitate discussions among family members regarding care decisions, promoting effective communication and conflict resolution.

Qualifications\n
