Key Responsibilities\n

• Conduct comprehensive psychosocial assessments to evaluate clients' mental health needs, strengths, and available resources.

• Provide individual, family, and group therapy by employing various evidence-based treatment modalities, such as cognitive-behavioral therapy and trauma-focused therapy.

• Develop individualized treatment plans that outline specific therapeutic goals and interventions tailored to each client's unique circumstances.

• Collaborate with a multidisciplinary treatment team, including psychiatrists, psychologists, and other healthcare professionals, to ensure a coordinated approach to care.

• Facilitate crisis intervention, risk assessments, and safety planning to address immediate concerns for clients in distress.

• Maintain thorough and accurate documentation of client progress in compliance with ethical and legal standards, using electronic health record systems.

• Facilitate group therapy sessions and psychoeducation classes to promote resilience and coping strategies among clients and their families.

Patient Population / Client Focus\n

As a Licensed Clinical Social Worker in our hospice setting, you will work closely with individuals facing end-of-life issues and their families. Qualifications\n

• Master's degree in Social Work from an accredited program.
