Key Responsibilities\n

• Conduct comprehensive psychosocial assessments to identify patients' mental health needs, strengths, and available resources.

• Provide individual and family therapy, implementing evidence-based treatment modalities such as cognitive-behavioral therapy and solution-focused therapy tailored to hospice care.

• Develop and document individualized treatment plans, setting realistic goals and determining therapeutic interventions aligned with patients’ care plans.

• Facilitate group therapy sessions and psychoeducation classes aimed at enhancing coping strategies concerning grief, loss, and terminal illness.

• Engage in crisis intervention and risk assessment to ensure patient safety and well-being during emotionally turbulent times.

• Collaborate with a multidisciplinary team, including medical professionals such as physicians and nurses, to coordinate comprehensive patient care.

• Maintain clear, up-to-date documentation of patient care in compliance with ethical and legal standards and regulations.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution.
