Work Environment

This position is primarily onsite, where you will engage with patients directly within their homes. You will collaborate with an interdisciplinary team of healthcare professionals, including nurses, social workers, and occupational therapists, to ensure a holistic approach to patient care. Key Responsibilities

• Deliver comprehensive care to a varied caseload, including adults and pediatric patients, ensuring alignment with patients' unique needs and established standards of practice.
• Conduct precise evaluations of patients' physical capabilities and limitations, using established assessment tools, tests, and measures to set and achieve therapy goals effectively.
• Collaborate with an interdisciplinary team to develop individualized treatment plans and engage in regular case discussions, enhancing patient care outcomes.
• Document all patient interactions meticulously within our electronic medical record system, ensuring proper billing charges are recorded accurately and timely.
• Perform screening evaluations for lymphedema, fall risks, and the necessity for rehabilitation services, promoting preventative care measures for our clients.
• Utilize specialized therapeutic techniques, including trigger point therapy, to help improve mobility, relieve pain, and support the restoration of patients' function.

Patient Population / Client Focus

In this role, you will work with a diverse range of patients, including those with physical injuries, chronic pain conditions, developmental disabilities, and mental health challenges. Your approach will be tailored to each individual, ensuring that therapeutic interventions address both physical rehabilitation and psychosocial support. This focus on holistic care aims to empower patients to regain their independence and enhance their quality of life.

Qualifications

• Active Florida Physical Therapist License.
• Bachelor's degree in Physical Therapy from an accredited program; a Master's or Doctoral degree is preferred for advanced practice.
• Minimum of one-year clinical experience in a healthcare setting, particularly in acute care hospitals.
• Proven ability and experience working with pediatric patients.
• Strong foundational knowledge in human anatomy and physiology, with competency in physical therapy practices.
• Familiarity with hospital medicine practices and comfort in both clinical and hospital environments is essential for this role.

Preferred Experience

• Experience with trigger point therapy techniques for effective pain management.
• Previous involvement in interdisciplinary team settings or collaborative care models is a plus.
• Demonstrated ability to mentor or supervise students/interns in a clinical environment.

Compensation and Benefits

The position offers an attractive annual salary ranging from USD 120,000 to 130,000, commensurate with experience and qualifications. In addition to competitive pay, we provide:
