You will work primarily in a home care setting, providing services directly in patients' residences in the Boca Raton area. Key Responsibilities

• Develop and execute personalized therapy plans for each patient, emphasizing early intervention techniques and timely evaluations to foster optimal recovery.
• Collaborate effectively with fellow therapists and clinicians to create a professional working ambiance that champions teamwork, quality care, and patient safety.
• Analyze clinical practices by partnering with the Director of Therapy Operations (DTO) to identify and implement improvement opportunities.
• Engage with referral sources including employers and insurance payers to strengthen relationships and enhance streamlining of communication regarding patient care.
• Utilize advanced clinical systems for efficient documentation, appropriate coding levels, and comprehensive charge capture to ensure all patient care orders are executed smoothly.
• Maintain strict compliance with the Florida State Practice Act and relevant regulations to ensure safe and ethical practice.
• Monitor patient feedback and engagement metrics to elevate the patient experience and satisfaction levels consistently.
• Integrate components of the Physical Therapy Guidebook into daily practice to ensure that every patient visit meets the highest standards of care.
• Oversee clinical outcomes and address complex cases collaboratively with fellow therapists to refine treatment plans.
• Streamline clinical workflows to enhance patient capture rates and enhance efficacy in processing referrals and scheduling.

Patient Population / Client Focus

As a Licensed Physical Therapist at Revive, you will work closely with a diverse range of patients requiring rehabilitation due to physical impairments from injuries, surgeries, or chronic conditions. Qualifications

• Active state licensure as a Physical Therapist in Florida.
• A Bachelor's, Master's, or Doctorate degree in Physical Therapy from an accredited institution.
