Key Responsibilities

• Conduct thorough evaluations of patients' physical conditions, which includes reviewing medical histories, performing assessments, and documenting findings to develop tailored treatment plans.
• Clearly communicate evaluation results and treatment strategies to patients and their families, actively involving them in the goal-setting process for better engagement and adherence.
• Implement treatment protocols effectively based on evaluation outcomes and adapt them as necessary to meet the evolving needs of each patient.
• Collaborate with a multidisciplinary healthcare team, consulting with other professionals when necessary and documenting any changes in the patient's condition that may require adjustments to their care plan.
• Manage discharge planning and follow-up care to ensure a seamless transition for patients, while maintaining precise and timely documentation in accordance with our evolving healthcare standards.
• Exhibit excellent time management skills, meeting productivity expectations, maintaining an accurate schedule, and ensuring prompt attendance at meetings.
• Engage in departmental goals and contribute to ongoing improvement initiatives, ensuring compliance with relevant regulations and standards.
• Commit to ongoing professional development, pursuing continuing education opportunities both individually and as part of a collaborative team environment, and serve as a mentor to students in clinical training.
• Promote an outstanding customer experience through flexibility and adaptability, ensuring patients receive the highest standards of care.
• Participate in initiatives aimed at improving access to care, understanding site-specific performance metrics, and working collaboratively to remove barriers faced by patients.

Patient Population / Client Focus

As a Physical Therapist, you will work closely with a diverse patient population, including those recovering from surgery, managing chronic pain, or undergoing rehabilitation for various medical conditions. Our goal is to provide customized care that accommodates each patient's unique situation, empowering them to achieve their personal health goals.

Qualifications

• Must hold a Doctorate Degree in Physical Therapy (DPT) and possess a valid state license in Florida.
• American Heart Association's Basic Life Support (BLS) for Healthcare Providers certification is required.
• Up-to-date training and certifications relevant to home healthcare practices.
• Demonstrated commitment to professional growth and continuous education.

Preferred Experience

• While prior work experience as a Physical Therapist is valued, a minimum of 2 years in a clinical setting is preferred.
• Experience working in home healthcare environments can give candidates a competitive advantage.

Compensation and Benefits
