Work Environment

This role is based on-site within our home care facility in Port Saint Lucie, Florida. Key Responsibilities

• Conduct thorough assessments of patients' physical conditions and capabilities to establish personalized treatment plans that meet their unique recovery goals.
• Deliver hands-on therapy that may include exercises, manual therapy, and the use of specialized equipment to improve strength, flexibility, and overall physical function.
• Collaborate with physicians and other healthcare professionals to ensure cohesive and effective treatment strategies are implemented.
• Monitor patient progress diligently, making necessary adjustments to treatment protocols based on their development and response.
• Educate patients and their families on injury prevention, home exercises, and health management to facilitate long-term wellness.
• Maintain accurate, up-to-date patient records and documentation in compliance with regulatory standards and organizational policies.
• Participate in regular team meetings and case reviews to share insights and improve treatment offerings.

Patient Population / Client Focus

As a Physical Therapist at our facility, you will primarily work with patients recovering from orthopedic injuries, surgeries, and chronic pain conditions. Qualifications

Compensation and Benefits
