Key Responsibilities\n

• Act as a liaison between clients, families, and outpatient providers to ensure continuity of care and effective communication.

• Engage with insurance companies for pre-certification, ensuring all necessary documentation and authorizations are completed accurately.

• Participate in discharge and aftercare planning to facilitate a smooth transition for clients returning to their daily lives, while providing ongoing support as needed.

• Deliver therapeutic meal support, guiding clients in establishing a positive relationship with food and nutrition as part of their recovery process.
