Key Responsibilities\n

• Establish quick rapport and communicate effectively with patients, their families, and team members, employing active listening skills while translating complex information into understandable terms.

• Function collaboratively as part of a professional team that includes behavioral health providers, primary care teams, and other relevant stakeholders.

• Conduct brief, evidence-based assessments of mental health conditions, screen for concerns such as depression, anxiety, and substance abuse, and create individualized treatment plans based on those assessments.

• Develop and maintain a personalized patient caseload, providing ongoing support and follow-up interventions, and ensuring continuity of care.

• Utilize electronic medical records (EMR) efficiently, completing documentation within 48 hours following patient encounters to support integrated healthcare delivery.
\n
• Design and implement comprehensive care plans, coordinating with various service providers while ensuring compliance with patient privacy and confidentiality regulations.
\n
• Engage actively in clinical supervision, team meetings, case conferences, and quality improvement initiatives, contributing your insights and expertise to enhance care delivery.
\n
• Respond promptly to patient inquiries and coordinate referrals to specialized services, including psychiatric consultations when medication is prescribed.
\n
• Identify community resources and assist patients in linking to additional support services that align with their personal goals.
\n
• Perform other duties as assigned to support departmental objectives and patient care.

Compensation and Benefits\n

The salary range for this position is competitive, offering an annual income of USD 90,000 to 100,000.
