Key Responsibilities\n

• Communicate effectively with physicians to report on patient progress and address specific needs and concerns, ensuring a holistic approach to the patient's care.

• Collaborate with clients' families and support systems to create comprehensive treatment plans and facilitate effective discharge planning.

• Maintain meticulous records, including clinical and progress notes, adhering strictly to departmental policies and legal regulations.
\n
• Work alongside healthcare team members to develop individualized treatment plans based on psychosocial assessments, incorporating evidence-based social work interventions.

• Assist clients with the discharge process, coordinating care and facilitating access to a wide array of regional services needed for successful transitions.

This role involves working with a diverse client population, including individuals facing challenges related to mental health disorders and substance abuse. Our clients vary in ages, backgrounds, and treatment needs, providing a rich environment for therapeutic engagement and impact. Qualifications\n

• A Master’s degree in Social Work, Counseling, Marriage and Family Therapy, or Psychology is required.

• Current licensure as an LCSW, LMFT, LPC, or an equivalent credential in the state of Pennsylvania is essential.

• A minimum of three years of clinical experience in an outpatient setting is mandatory, with a demonstrated ability to work with diverse populations.

• Advanced computer skills are necessary, particularly in word processing, email communication, spreadsheet management, and familiarity with electronic medical record (EMR) systems.

• Capable of working independently while being highly organized and self-directed to manage your caseload effectively.
