Work Environment

This role is located in a supportive, fast-paced outpatient setting in Glen Cove, NY, where you will work alongside a multidisciplinary team of healthcare providers. The work environment promotes ongoing education and engagement through community outreach events. As a Family Nurse Practitioner, you will typically work from 9:00 a.m. to 6:00 p.m., with occasional evening clinics until 8:00 p.m., and an average work week of around 40 hours. Participation in community events or health fairs on weekends may be required.

Key Responsibilities

• Perform thorough health assessments and screenings for both individuals and families, identifying healthcare needs and establishing individualized care plans.
• Diagnose and manage acute and chronic conditions, facilitating preventive care measures and promoting health literacy among patients.
• Engage with local organizations and stakeholders to design and implement impactful health education programs that address community needs.
• Provide culturally competent care that respects diverse backgrounds and advocates for the patient's rights and access to healthcare services.
• Coordinate patient referrals and ensure follow-up care to guarantee continuity of care and optimal health outcomes.
• Maintain accurate and detailed medical records in adherence to legal and organizational standards, facilitating quality patient care.
• Contribute to community outreach initiatives, facilitating wellness screenings that elevate public health awareness and prevention.
• Stay informed on community health trends, advancements in evidence-based practices, and new healthcare policies to enhance service delivery.

Patient Population / Client Focus

You will primarily serve the Glen Cove community, focusing on providing care to diverse populations with a particular emphasis on those facing socioeconomic barriers. Your role involves assessing and addressing health disparities, tailoring interventions that meet the needs of our patients, and fostering trusting relationships that encourage active participation in their health journeys.

Qualifications

• Current certification as a Family Nurse Practitioner.
• An active nursing license in the state of New York.
• A Master's degree in nursing from an accredited program.
• A minimum of 3-5 years of clinical experience in family practice, community health, or public health settings preferred.
• Exemplary interpersonal and communication skills, capable of building rapport with patients from various cultural backgrounds.

Preferred Experience
