Key Responsibilities\n

• Conduct thorough assessments of clients to Identify their unique social, emotional, and environmental needs.

• Develop tailored service plans that set clear goals and interventions, ensuring continuous monitoring of client progress.

• Provide individual and family counseling to guide clients through life’s challenges, fostering resilience and personal growth.

• Assist clients in accessing appropriate community resources, including shelters, rehabilitation programs, and support groups.

• Offer immediate crisis intervention and support, conducting risk assessments and crafting safety plans during emergencies.

• Collaborate with local agencies to ensure a seamless delivery of services and advocate vigorously on behalf of clients.

• Participate in community outreach initiatives to promote agency services and build awareness around prevalent social issues.

• Engage in regular team discussions and training sessions to identify best practices and enhance service provision.

Patient Population / Client Focus\n

You will predominantly serve individuals and families facing adversity due to mental health issues, socioeconomic instability, and other social determinants of health. Your work will focus on empowering these populations to overcome barriers and achieve improved well-being through personalized interventions and community resource connections.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution.

• An active state-issued license as a Licensed Master Social Worker (LMSW).
