Key Responsibilities

• Conduct thorough evaluations of patients to accurately assess their physical abilities, limitations, and therapy needs.
• Design personalized treatment plans tailored to enhance patient strength, mobility, and functional independence.
• Implement various therapeutic exercises, manual therapy techniques, and modalities such as ultrasound and electrical stimulation to treat specific physical impairments.
• Monitor and document the ongoing progress of each patient, making necessary revisions to treatment plans based on their development and response to therapy.
• Engage in interdisciplinary collaboration by communicating effectively with team members, including case conferences, to ensure comprehensive patient care.
• Educate patients and their families on preventative strategies and home exercise regimens that promote long-term health and wellness.
• Maintain compliance with state regulations, professional standards, and clinic policies to uphold the highest standards of care.

Patient Population / Client Focus

This position primarily focuses on the outpatient geriatric population, including those with musculoskeletal disorders, post-operative rehabilitation, and chronic pain conditions. You will engage with patients who require specialized attention to regain their independence and enhance their physical capabilities. Understanding the unique needs of elderly patients, you will develop strategies that empower them to take charge of their health.

Qualifications

• A valid Physical Therapist license in the state of New York is mandatory.
• A Bachelor's Degree in Physical Therapy from an accredited institution is required; however, candidates with a Master's Degree or Doctorate in Physical Therapy will be given preference.
• Strong communication and interpersonal skills, with an ability to connect with patients and their families effectively.
• Excellent organizational skills to manage multiple cases and maintain accurate documentation.
• Commitment to professional development through continuous learning opportunities and adherence to best practices.

Preferred Experience

• At least two years of experience working as a Physical Therapist, preferably in an outpatient or skilled nursing facility.
• Experience in geriatric physical therapy, particularly in developing care plans for elderly patients.
• Familiarity with electronic medical record (EMR) systems to document patient information efficiently and accurately.
• Engagement in community outreach or educational programs to promote health and wellness is a plus.

Compensation and Benefits

This position offers a competitive salary ranging from USD 90,000 to 110,000 annually, commensurate with experience and qualifications.
