Work Environment\n

The role offers a flexible remote work setting, allowing you to meet clients where they feel most comfortable while maintaining efficient communication with our interdisciplinary team. Our work hours are Monday to Friday, from 8:00 AM to 4:30 PM, with occasional evenings and weekends as needed to accommodate client schedules or urgent situations. The nature of hospice care emphasizes the importance of on-call availability for critical support. You will work collaboratively with skilled healthcare professionals, including nurses and caregivers, creating a robust network of care dedicated to enhancing the lives of seniors.

Key Responsibilities\n

• Conduct detailed assessments of clients' psychosocial needs, evaluating emotional, cognitive, and social factors to inform care strategies.

• Develop individualized care plans in collaboration with clients and their families, ensuring the goals align with their preferences and circumstances.

• Provide one-on-one counseling and emotional support to seniors and family members, assisting them in navigating life transitions and bereavement processes.

Qualifications\n

• A Master’s degree in Social Work (MSW) from an accredited institution.

• Current and valid licensure as a Licensed Clinical Social Worker (LCSW) in Texas.

Compensation and Benefits\n
