Work Environment

This role is based onsite, providing home care to patients throughout San Jose and the surrounding areas. The work environment allows physical therapists to engage deeply with patients in a comfortable and familiar setting, enhancing the therapeutic experience. Key Responsibilities

• Conduct thorough patient assessments and develop tailored rehabilitation plans based on individual goals and medical histories.
• Engage with 1-2 patients per hour, delivering focused treatment sessions that employ evidence-based practices and hands-on techniques.
• Regularly monitor patient progress and adapt treatment protocols to optimize outcomes and address evolving needs.
• Utilize WebPT to maintain comprehensive patient records, documenting treatment plans and results while collaborating seamlessly with the multidisciplinary care team.
• Educate patients and their families about exercises, home care strategies, and lifestyle modifications to enhance recovery.

Patient Population / Client Focus

Our patient population primarily consists of individuals recovering from orthopedic injuries, surgeries, or chronic conditions. As a physical therapist, you will work closely with a diverse range of patients, providing personalized treatment that addresses both physical and emotional health, fostering a holistic approach to recovery.

Qualifications

• A Bachelor's degree in Physical Therapy from an accredited institution is required; a Master's or Doctorate degree is preferred.
• Possession of an active California Physical Therapist license or the ability to obtain one prior to starting the role.
• Demonstrated clinical reasoning skills and a commitment to hands-on rehabilitation practices that yield measurable patient benefits.
• Strong interpersonal and communication skills, essential for motivating patients and fostering positive relationships.

Preferred Experience

• A minimum of 2 years of clinical experience in physical therapy is required, with a preference for backgrounds in home health care or outpatient therapy.
• Experience working with orthopedic conditions and postoperative rehabilitation is highly desirable.
• Familiarity with using electronic health records, particularly WebPT or similar platforms, is a plus.

Compensation and Benefits

On Point Physical Health Therapy offers a competitive annual salary range of USD 120,000 to 130,000, commensurate with experience.
