Licensed Physical Therapist for Home Care Services

Work Environment

This position will predominantly involve home care settings, allowing you to work one-on-one with patients in their own environments. You will collaborate closely with our interdisciplinary team to create a supportive and effective treatment landscape. Key Responsibilities

• Conduct thorough evaluations and assessments to develop individualized treatment programs for patients referred by physicians.
• Execute a variety of therapeutic interventions at patients' homes, using advanced clinical techniques to improve their physical capabilities, mobility, and overall well-being.
• Work collaboratively with a diverse team, including other healthcare professionals, to ensure that patient care is cohesive and comprehensive.
• Document patient progress and outcomes, maintaining meticulous records that align with best practices and regulatory requirements.
• Educate and train patients and caregivers on home exercise programs, adaptive techniques, and preventive strategies to facilitate ongoing recovery.
• Meet or exceed minimum utilization goals and other business performance objectives while ensuring high levels of patient satisfaction.

Patient Population / Client Focus

Our patient demographic encompasses a wide range of specialties, including orthopedic, pediatric, neurologic, and sports medicine. Qualifications

• Graduate of an APTA or CAPTE-approved physical therapy program.
• Must possess a Doctorate in Physical Therapy.
• Active state license in Physical Therapy.
• Basic Life Support Certification from the American Heart Association.
• Excellent written and verbal communication skills, along with proficient computer and word processing capabilities.

Preferred Experience

Prior experience in home care or community-based settings is highly desirable. Candidates who demonstrate a proactive approach to patient care, alongside a passion for building relationships, will excel in this role. Compensation and Benefits

The position offers a competitive salary range of USD 90,000 to 110,000 annually, based on experience and qualifications. In addition to monetary compensation, we provide a robust benefits package that includes:
