Work Environment

As a Physical Therapist within our home care setting, you will work onsite, directly with patients in their own environments. This approach helps you to gain insights into the patient's daily lives and tailor treatments that fit seamlessly into their routines. Your weekly schedule of 40 hours includes variable weekends, allowing for flexible patient care. Our clinic emphasizes teamwork, fostering relationships with an interdisciplinary treatment team that includes physicians, social workers, and other health professionals.

Key Responsibilities

• Conduct comprehensive examinations and evaluations of patients to identify physical limitations and devise suitable treatment plans.
• Consult with healthcare providers regarding patient care continuity and adapt treatment plans as necessary.
• Maintain thorough documentation of treatment processes and outcomes according to applicable laws and professional standards.
• Effectively communicate and collaborate with interdisciplinary teams, including patient families, to ensure cohesive care and discharge planning.
• Supervise and mentor assigned support staff, enhancing their skills and promoting best practices in patient care.
• Participate in community outreach events and contribute to the development and improvement of our therapy programs.
• Engage in ongoing professional development through continuing education, in-services, and self-study to maintain expertise in the field.

Patient Population / Client Focus

Your role as a Physical Therapist will involve working closely with a diverse patient population ranging from pediatric patients to seniors facing various functional limitations and disabilities. You will provide compassionate care tailored to individual needs, helping patients achieve optimal recovery and quality of life.

Qualifications

• Bachelor's Degree in Physical Therapy from an accredited institution.
• Current state licensure in Physical Therapy.
• BLS certification.
• Strong verbal and written communication skills, alongside proficiency in word processing and electronic medical record systems.

Preferred Experience

• Prior experience in a home care setting or with similar populations is highly regarded.
• Ability to establish rapport with patients from diverse cultural backgrounds.
• Experience collaborating in an interdisciplinary team environment is a plus.

Compensation and Benefits
