Med Lake Chiropractic and Physical Therapy is seeking a dedicated and experienced Licensed Physical Therapist to join our team in Santa Clara, California. Work Environment

You will be working onsite at our Santa Clara outpatient office, which is designed to create a welcoming space for patients. Our facility is equipped with state-of-the-art rehabilitation equipment and resources, allowing you to deliver the best possible therapeutic interventions. The clinic's atmosphere is collaborative, encouraging continuous professional development and teamwork among our staff. Work hours are Monday to Friday from 9 AM to 5 PM, providing a balanced work-life schedule.

Key Responsibilities

• Conduct thorough evaluations and assessments of patients to understand the nature and extent of physical impairments, dysfunctions, or disabilities.
• Develop and implement individualized treatment plans that align with patients' goals and considerations, factoring in their medical history and functional challenges.
• Monitor and document patient progress, adjusting treatment plans as necessary based on evidence-based practices and regulatory requirements.
• Communicate effectively with patients, their families, and other healthcare team members regarding care progress, issues, or changes.
• Maintain accurate patient records and documentation to fulfill licensing and accrediting standards.
• Instruct patients' families and nursing staff on care plans and home exercise programs to ensure continuity of care.
• Ensure that the clinic and equipment are maintained in a safe and clean condition to promote a healthy environment for both patients and staff.

Patient Population / Client Focus

Our patient population includes individuals dealing with a range of conditions, from orthopedic injuries to sports-related rehabilitation and chronic musculoskeletal issues. You will work closely with a diverse clientele, developing treatment plans that address unique patient needs and supporting them in their rehabilitation journey. Qualifications

• A Bachelor's Degree in Physical Therapy from an accredited program.
• Current California State licensure for the practice of physical therapy is required.
• National Provider Identifier (NPI) is mandatory.
• Strong foundation in evidence-based treatment techniques and methodologies.
• Exceptional communication and patient education skills, with the ability to motivate patients to adhere to their treatment protocols.
• Proven ability to work both independently and collaboratively within a multidisciplinary team environment.

Preferred Experience

• Minimum of 2 years of experience in orthopedic and sports rehabilitation settings.
• Experience working with a diverse patient population and a variety of musculoskeletal conditions.
• Knowledge of current best practices and ability to implement effective therapeutic exercises tailored to individual patient needs.

Compensation and Benefits
