Key Responsibilities\n

As a Senior Licensed Clinical Social Worker, your key responsibilities will include:

• Conducting in-depth psychosocial assessments to evaluate patient needs, strengths, and areas for improvement, ensuring each client receives tailored support.

• Providing high-quality counseling services, focusing on disease management and health education to empower patients in their mental health journeys.

• Collaborating with patients and an interdisciplinary healthcare team to design and implement customized care plans aimed at behavioral modification and lifestyle changes.

• Setting individualized goals with patients, guiding them toward better management of their chronic health conditions and associated mental health challenges, both synchronously and asynchronously.

• Accurately documenting patient interactions, progress, and care plans in our EHR system in a timely and organized manner to ensure seamless continuity of care.

Qualifications\n
