Key Responsibilities\n

• Foster meaningful relationships with patients, families, and healthcare professionals to create and implement personalized care plans that address psychosocial needs, ensuring that all aspects of care are integrated and respectful of the patient’s wishes.

• Conduct thorough assessments and maintain detailed documentation of patient and family interactions, employing the latest documentation standards to ensure accuracy and compliance with state regulations and organizational policies.

• Manage and address conflicts or resistance from clients by employing therapeutic techniques to diffuse tension and facilitate constructive dialogue, promoting healthy coping mechanisms and behaviors.

• Stay current with the latest DSM diagnostic criteria and demonstrate proficiency in understanding developmental tasks across different age groups, applying this knowledge to support children, adolescents, and their families within the context of hospice care.

• Implement evidence-based therapeutic approaches, including behavior modification and supportive therapy, tailoring your interventions to meet the unique needs of each client.

As a Senior LCSW, you will specifically work with patients facing terminal illnesses and their families, providing critical support during one of the most challenging times in their lives. Qualifications\n
