Key Responsibilities\n

• Provide compassionate, goal-directed counseling to patients, especially those seeking transplant services, addressing both emotional and psychological needs.

• Assess patient awareness and preferences regarding advance directives, facilitating necessary discussions with both the healthcare team and supportive family members.

• Actively collaborate with an interdisciplinary team to conduct comprehensive psychosocial assessments, identifying patient strengths and areas requiring intervention.

• Educate patients and their families on community resources, helping them connect with necessary support services such as vocational rehabilitation and home health assistance to optimize treatment outcomes.

• Develop personalized care plans with patients, focusing on effective therapies that improve their quality of life and meet rehabilitation goals.

Qualifications\n

Compensation and Benefits\n
