Your contributions will be integral to the Community Health Clinic model, which emphasizes team collaboration and comprehensive care.

Key Responsibilities\n

• Conduct thorough psychosocial assessments to identify the mental health needs of clients effectively.

• Develop individualized care plans that incorporate evidence-based therapeutic techniques.
\n
• Facilitate individual and group therapy sessions, employing a variety of therapeutic modalities to promote healing.

• Collaborate with a multidisciplinary team of healthcare professionals, including psychiatrists, nurses, and community service providers, to ensure holistic support for clients.

• Advocate for clients in accessing community resources, ensuring they can obtain necessary services such as housing, employment, and additional mental health supports.

• Maintain detailed documentation of client interactions, treatment progress, and care plans using electronic health records to facilitate continuity of care.

• Participate in ongoing training, team meetings, and clinical supervision to enhance your clinical skills and contribute to peer education.

Patient Population / Client Focus\n

You will primarily work with adults, including older adults, who may be facing challenges such as depression, anxiety, trauma, or life transitions. Your focus will be on fostering resilience and supporting clients through their mental health journeys, with particular attention to the unique needs of the diverse populations in Ontario, California.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution.

• Current licensure as a Licensed Clinical Social Worker (LCSW) in the state of California.

• A minimum of two years of clinical experience, preferably with older adults or in community health settings.
\n
• Demonstrated clinical assessment, diagnostic, and intervention skills.
\n
• Excellent verbal and written communication skills, along with a genuine compassion for others.
\n
• Ability to work independently while also being an integral member of a collaborative team.

Preferred Experience\n

Compensation and Benefits\n
