Work Environment

As part of our onsite team, you will work primarily in the patients' homes, facilitating comfort and ease of treatment. Key Responsibilities

• Conduct comprehensive evaluations, including assessments of physical function and mobility, to create personalized treatment plans that consider each patient's individual needs and health history.
• Clearly communicate evaluation results and collaboratively develop treatment objectives with patients and their families, fostering a sense of involvement in their care journey.
• Implement evidence-based treatment protocols, ensuring alignment with evolving healthcare standards, and regularly update care plans based on patient progress and feedback.
• Consult with interdisciplinary healthcare teams when necessary to enhance patient care and address any complications or changes in health status.
• Manage documentation with precision, including patient progress notes, discharge summaries, and other reports that reflect improved outcomes and align with care goals.
• Demonstrate effective time management by maintaining an organized schedule, attending meetings punctually, and ensuring compliance with both clinic goals and regulatory standards.
• Engage in community marketing initiatives that promote Tri-County Physical Therapy's services and foster positive relationships within the community.
• Embrace a proactive approach to pursuing continuing education and professional development to enhance skills and knowledge within the field.
• Enhance the overall patient experience by being adaptable and responsive to patient needs, improving access to services.
• Participate in removing barriers to healthcare access and analyze site performance data to actively contribute to the Access NRH initiatives.

Patient Population / Client Focus

You will primarily work with diverse populations varied by age, diagnosis, and functional ability, ensuring that each patient receives tailored treatment that addresses their unique challenges and health goals. Your role will be crucial in helping individuals regain mobility, reduce pain, and improve their quality of life through targeted intervention strategies and compassionate care.

Qualifications

• A Bachelor's Degree in Physical Therapy from an accredited institution is required.
• A valid license to practice as a Physical Therapist in Florida, or eligibility for licensure.
• American Heart Association's Basic Life Support (BLS) certification is required for all healthcare providers.
• While previous experience in a similar role is not required, a passion for patient care and a willingness to learn are essential.

Preferred Experience
