Key Responsibilities\n

• Conduct comprehensive patient intakes, including diagnostic and functional evaluations, providing insightful recommendations to the primary care team.

• Administer brief, focused interventions for patients experiencing mental health crises, ensuring timely and appropriate care.

• Work in conjunction with primary care providers to identify patients who may require referrals to specialized mental health facilities.

• Regularly assess patient progress, initiating follow-up outreach to evaluate therapeutic efficacy and make necessary adjustments to treatment plans.

• Develop and implement relapse prevention plans for patients at risk of exacerbating their conditions.
\n
• Monitor the coordination of behavioral health services, making strategic connections with additional treatment providers as needed, with patient consent.

• Maintain accurate clinical documentation and progress notes, ensuring full compliance with Agency and Federally Qualified Health Center (FQHC) regulations.

• Educate patients on self-management skills, empowering them to take active roles in their health and recovery processes.

• Participate in clinic management discussions to provide feedback on behavioral health services, contributing to continuous improvement of care practices.

• Assist patients in adhering to medical treatment recommendations from primary care providers, offering support for managing medication side effects.

Qualifications\n

• A Master's Degree in Clinical Counseling is required.
