Hospice Social Worker

RVNAhealth is seeking a compassionate and experienced Hospice Social Worker (LCSW or LMSW) to join our Hospice and Palliative Care team. This field-based role provides psychosocial, emotional, and supportive services to hospice and pre-hospice patients and their families throughout Southern Fairfield County. The Hospice Social Worker is an integral member of the interdisciplinary team, helping patients and families navigate the emotional and practical challenges of serious illness, end-of-life care, grief, and bereavement. Schedule: Full time. Monday–Friday, 8:30am - 4:30pm (37.5 hours/week) Setting: Field-based; patient visits in homes Territory: Southern Fairfield County, including Norwalk, Westport, Darien, Stamford, Greenwich, and surrounding communitie Key Responsibilities Conduct initial and ongoing psychosocial, emotional, spiritual, and bereavement assessments. Develop and implement individualized care plans in collaboration with the Hospice interdisciplinary team. Provide counseling, crisis intervention, emotional support, and caregiver support. Assist patients and families with end-of-life planning, advance directives, living wills, and powers of attorney. Serve as a patient and family advocate and connect families with appropriate community resources. Assess suicide risk and develop appropriate care plans when indicated. Participate in interdisciplinary team meetings, case conferences, bereavement activities, and community education. Collaborate with nurses, physicians, chaplains, volunteers, and other members of the care team. Maintain timely and comprehensive electronic medical record documentation, including point-of-care documentation. Participate in quality improvement activities, professional development, and the Hospice on-call rotation. Follow all agency policies and applicable Hospice regulatory and compliance requirements. Qualifications Master's degree in Social Work from a CSWE-accredited program. Current Connecticut LCSW or LMSW license . Minimum 5 years of social work experience following completion of the master's degree . Minimum 1 year of healthcare experience required . Hospice or end-of-life care experience preferred. Experience providing psychosocial assessment, counseling, care planning, advocacy, and family support. Strong communication, clinical judgment, crisis intervention, and interpersonal skills. Ability to work independently while collaborating effectively with an interdisciplinary team. Comfortable working with diverse patient and family populations. Computer proficiency and ability to document at the point of care. Valid driver's license and ability to travel throughout the assigned territory in varying weather and environmental conditions. Compensation The pay range listed represents RVNAhealth's commitment to pay transparency and is a reasonable estimate of possible compensation for this role at the time of posting. The final salary for this role will be determined based on careful consideration of a variety of factors, such as the candidate's exact location, experience, and expertise related to the qualifications.
