Key Responsibilities\n

• Conduct thorough biopsychosocial assessments to identify patient needs, incorporating medical, emotional, and social history into treatment planning.

• Deliver tailored therapeutic interventions, both short-term and long-term, that are specific to the healthcare context and align with clinical best practices.

• Coordinate patient care and effective discharge planning with medical staff and community resources, ensuring seamless transitions of care.

• Help patients and their families navigate complex healthcare systems, providing guidance on insurance options, financial aid, and access to essential social services.

• Lead and facilitate support groups and psychoeducational workshops, fostering a safe space for participants to share experiences concerning mental health.

• Address social determinants of health affecting recovery such as housing, employment, and transportation, using the appropriate community resources.

• Advocate for patient rights, ensuring we provide culturally competent, individualized care that respects diverse backgrounds and values.

• Maintain accurate and confidential documentation in accordance with healthcare regulations and organizational policies, using electronic health record (EHR) systems.

• Participate actively in interdisciplinary team meetings, case conferences, and ongoing professional development to remain at the forefront of mental health practices.

Patient Population / Client Focus\n

This role primarily supports adults and adolescents dealing with a variety of mental health issues, including anxiety, depression, trauma, and substance abuse disorders. Qualifications\n

• Master's Degree in Social Work from an accredited institution is required.

• Active LCSW license or LMFT issued in the state of California.

• Proficient in Medicaid and Medicare regulations, as well as private insurance processes.

• Capacity to be credentialed with various insurance providers and obtain necessary NPI numbers.

• Demonstrated ability to develop and execute functional treatment plans that reflect patient preferences and goals.

• Possess personal/professional liability insurance.

Preferred Experience\n

• A minimum of two years of clinical experience providing mental health services to a diverse patient population, preferably in an outpatient or hospital setting.

Compensation and Benefits\n
