Key Responsibilities

• Conduct thorough evaluations of patients recovering from injuries, surgeries, or chronic conditions within their home settings.
• Develop personalized treatment plans utilizing evidence-based practices, including therapeutic exercises, manual therapy techniques, and modalities to alleviate pain and improve mobility.
• Document all treatments, monitor patient progress, and make necessary adjustments to therapy goals and methodologies.
• Educate patients, families, and caregivers on proper techniques and use of assistive devices to promote independence and safety.
• Maintain current knowledge in the field by attending workshops, participating in professional organizations, and staying informed about industry advancements.

Patient Population / Client Focus

Your patient demographic will vary widely, including those recovering from orthopedic surgeries, stroke victims, individuals with arthritis, and the frail elderly. Qualifications

• A Doctorate in Physical Therapy (DPT) from an accredited institution.
• Current state license in Physical Therapy, permitting you to practice in New Jersey.

• Strong computer proficiency, particularly in electronic health record systems, to document and communicate patient care efficiently.
• A passion for patient care and the ability to adapt quickly to shifting circumstances in a home care setting.

Preferred Experience

While prior experience is not mandatory, candidates with at least two years of clinical experience in physical therapy, preferably in home care settings, will be favored. Your adaptability and experience with diverse patient systems can greatly enhance your effectiveness in this role.

Compensation and Benefits
