Key Responsibilities

• Conduct comprehensive evaluations of patients, review medical histories, perform assessments, and document results to inform individualized treatment plans.
• Engage patients and families in understanding the evaluation findings and collaboratively establish treatment goals tailored to each patient's needs.
• Implement effective treatment protocols based on evaluation findings, patient presentations, and best practices, while adjusting approaches to address evolving healthcare demands.
• Collaborate with other healthcare professionals, recognizing when changes in a patient's condition necessitate modifications to treatment plans or additional consultations.
• Handle discharge planning and subsequent follow-up care, ensuring timely and accurate documentation that reflects patients’ progress and treatment paths.
• Maintain efficient time management and productivity standards while meeting scheduled commitments, including being punctual for meetings and community outreach efforts.
• Contribute actively to departmental initiatives aiming for performance improvements, ensuring compliance with regulatory standards.
• Take the lead in pursuing continuing education opportunities for personal and professional growth while mentoring students as clinical instructors.
• Actively participate in initiatives to improve access to care for all patients, utilizing CSI measurement data to identify and dismantle barriers impeding service delivery.

Patient Population / Client Focus

As a Physical Therapist at Aviant Physical Therapy, you will serve a diverse patient population primarily focused on outpatient rehabilitation. Your expertise will apply to a range of conditions, including post-surgical recovery, chronic pain management, and mobility issues related to various medical diagnoses. This role emphasizes providing customized care that considers each patient's specific needs and circumstances.

Qualifications

• A Doctorate degree in Physical Therapy (DPT) is required.
• A valid State license for Physical Therapy (PT) in New Jersey.
• Certification in American Heart Association’s BLS for Healthcare Providers CPR is mandatory.
• Preferred candidates should be committed to maintaining their licensure and actively participate in necessary continuing education.

Preferred Experience

While 2 years of relevant experience in physical therapy is preferred, recent graduates are encouraged to apply as we value fresh perspectives and eagerness to learn.
