Work Environment

The work setting for this position is onsite, primarily involving direct home care visits to patients in need of rehabilitation services. Our office hours are Monday through Friday, from 8:00 AM to 5:00 PM, allowing for a consistent, full-time schedule with evenings and weekends off.

Key Responsibilities

As a Licensed Physical Therapist, you will be entrusted with multiple responsibilities that are central to patient care. Your day-to-day tasks will include:

• Conducting thorough patient evaluations to assess functional limitations and develop comprehensive treatment plans tailored to individual needs.
• Implementing and demonstrating effective treatment protocols that are informed by evaluation findings and patient conditions.
• Collaborating with healthcare team members to evaluate patient progress, making timely modifications to treatment plans as necessary based on changes in the patient's condition.
• Preparing detailed documentation that reflects the patient's treatment course, including initial evaluations, daily progress notes, interim reports, and discharge summaries.
• Engaging in consistent communication with other healthcare professionals to coordinate care and ensure an effective treatment approach.
• Participating in team meetings to address barriers to patient access and actively contribute to organizational initiatives.

Patient Population / Client Focus

You will work directly with a diverse patient population requiring physical therapy interventions for a variety of conditions. Your expertise will be essential in treating individuals with orthopedic injuries, post-surgical recovery, and those with functional limitations affecting their daily activities. You will utilize your skills to not only treat physical ailments but also to educate patients and their families about the recovery process, fostering a supportive recovery environment.

Qualifications

The ideal candidate will possess the following qualifications:

• A Doctorate of Physical Therapy (DPT) from an accredited physical therapy program.
• Current state licensure as a Physical Therapist.
• A minimum of two years of experience in outpatient orthopedic settings, demonstrating a strong understanding of treatment protocols and patient care.
• A valid CPR certification to ensure patient safety during therapy sessions.

Preferred Experience
