Work Environment

This position will be based onsite at patients' homes throughout the Woodbridge area, allowing you to provide personalized care in a supportive environment. You will work Monday through Friday, with a schedule designed to maximize your interaction with patients and streamline their rehabilitation process. Key Responsibilities

• Conduct comprehensive evaluations of patients, taking into account medical histories, specific assessments, and clinical findings to formulate effective treatment plans.
• Clearly articulate evaluation results and treatment strategies to patients and their families, engaging them actively in goal setting and care decisions.
• Implement evidence-based treatment protocols tailored to the individual patient's needs, adjusting strategies as required based on the patient’s progress and feedback.
• Collaborate proactively with other healthcare providers to optimize patient care, documenting any changes in a patient’s condition and adjusting treatment plans accordingly.
• Prepare patients for discharge by creating thorough and thoughtful discharge plans, ensuring continuity of care through appropriate follow-up communication.
• Maintain timely and accurate documentation of patient interactions and care provided, ensuring compliance with legal, accreditation, and regulatory standards.
• Exhibit strong time management skills to meet productivity expectations, while maintaining a flexible and adaptable approach to patient needs.
• Actively participate in community engagement and marketing efforts, representing Bountiful Life Physical Therapy in various outreach initiatives.
• Engage in continuous professional development by seeking educational opportunities and sharing knowledge with colleagues and interns.

Patient Population / Client Focus

The therapy services provided are primarily geared toward geriatric patients, individuals recovering from surgeries, and those managing chronic conditions. You will play an essential role in helping patients regain their independence and improve their quality of life through personalized rehabilitation strategies. Qualifications

• Doctorate in Physical Therapy (DPT) from an accredited institution.
• Current and valid physical therapy license in the State of New Jersey.
• CPR certification from the American Heart Association, specifically designed for Healthcare Providers.
• Strong interpersonal and communication skills for effective patient and family interactions.

Preferred Experience

While a minimum of two years of experience is preferred for this role, we are open to considering candidates with a genuine passion for physical therapy and a willingness to learn. Candidates with prior experience in home health environments or those who have undertaken specialized training in geriatric or post-surgical rehabilitation will be viewed favorably.

Compensation and Benefits
