Position Overview

Reliant Orthopedic and Sports is actively seeking a dedicated and experienced Physical Therapist (PT) to join our team in Newark, New Jersey. Work Environment

This role is positioned in the dynamic and rewarding landscape of home care, where you'll work directly within clients’ living environments. The onsite nature of this position allows you to tailor your approach based on each client’s unique surroundings, fostering an environment of healing and recovery. You will collaborate closely with patients, families, and interdisciplinary team members to deliver holistic care.

Key Responsibilities

• Develop and implement personalized treatment plans for clients to prevent disability, relieve pain, and restore muscle function.
• Oversee and guide Physical Therapy Assistants to ensure compliance with established care plans and protocols.
• Conduct detailed evaluations using diagnostic tests to assess clients' muscle, nerve, joint, and functional capabilities.
• Analyze the home environment and recommend adaptive strategies to enhance client independence.
• Deliver prescribed physical therapy services directly in clients’ homes, including administering therapeutic exercises, traction, and massage techniques.
• Evaluate, adjust, and fit prosthetic and orthotic devices according to clinical standards and patient needs.
• Engage in continuous collaboration with interdisciplinary teams during case conferences to optimize treatment strategies.
• Maintain accurate clinical documentation, including initial evaluations, progress notes, and discharge summaries, using EMR software such as Web PT.
• Educate clients and their families on physical therapy programs, home exercise plans, and the proper use of therapy devices.
• Conduct regular on-site functional assessments and revise care plans as necessary to adapt to clients’ changing conditions.
• Provide timely communication regarding changes in clients’ health status to physicians and case managers.
• Complete other assignments as required to achieve organizational goals and enhance patient care outcomes.

Patient Population / Client Focus

The ideal candidate will work with a diverse patient population, including individuals recovering from surgery, managing chronic conditions, or requiring rehabilitation following an injury. Qualifications

• A Doctorate in Physical Therapy (DPT) is required.
• An active Physical Therapist license in New Jersey, with no history of sanctions or board proceedings.
• Exceptional proficiency in various manual therapy techniques and exercise programming.
• Strong documentation skills and familiarity with EMR systems, particularly Web PT.
• Current knowledge of evidence-based physical therapy methods and practices.

Preferred Experience

• Prior experience in a home healthcare setting is highly valued.
• Demonstrated ability to work collaboratively in interdisciplinary teams.
• Experience in conducting functional assessments and creating individualized care plans.

Compensation and Benefits

Reliant Orthopedic and Sports offers a competitive salary between $100,000 and $120,000 annually, complemented by comprehensive benefits.
