Key Responsibilities\n

• Conduct comprehensive psychosocial assessments to identify the emotional, social, and financial needs of patients and their families, which may include gathering pertinent medical history and understanding family dynamics.

• Provide individual and group counseling sessions to patients, addressing issues such as adjustment to terminal diagnoses, grief, and coping strategies, utilizing evidence-based therapeutic techniques.

• Coordinate with community organizations and resources to arrange post-discharge services, ensuring that patients receive the continuity of care they need during transitioning phases.

• Offer emotional support and education to patients and families regarding available treatment options, community resources, and strategies for coping with illness.

Qualifications\n
