Key Responsibilities\n

• Perform comprehensive psychosocial assessments through detailed interviews and observations to understand patients' needs, capabilities, and behaviors.

• Deliver psychotherapeutic interventions aimed at addressing mental health issues such as depression, anxiety, grief, addiction, and chronic pain.

• Engage in crisis intervention when required, providing immediate support and guidance to patients and their families in stressful situations.

• Collaborate with interdisciplinary teams, including physicians and nursing staff, to develop and implement care plans that are responsive to the patients' psychological and emotional needs.

• Provide education and consultation to family members, equipping them with strategies and resources for coping with the realities of terminal illness and caregiving.

• Maintain accurate and timely documentation of patient interactions in compliance with healthcare regulations and organizational policies.

• Support discharge planning processes by working closely with hospice care teams to ensure a seamless transition for patients within the healthcare continuum.

The primary focus of this role will be patients receiving hospice care, which may include individuals with terminal diagnoses and their families. Qualifications\n

• A Master’s degree in Social Work (MSW) along with an active California state license as a Licensed Clinical Social Worker (LCSW).

This position offers a competitive annual salary ranging from $120,000 to $130,000, commensurate with experience and qualifications.
