Key Responsibilities\n

As a Licensed Clinical Social Worker at CHDC, you will be responsible for delivering a variety of mental health services, ensuring that our patients receive holistic care that addresses both behavioral and physical health needs. • Assist individuals navigating both physical and behavioral healthcare systems to promote integrated health care solutions tailored to each patient’s unique needs.

• Educate patients about available resources and health services, encouraging proactive engagement in wellness practices, disease prevention, and healthy lifestyles.

• Attend and actively participate in multi-disciplinary treatment team meetings to enhance collaborative care approaches and outcomes.

• Monitor and report all crisis incidents in a timely manner, including immediate communication with the Medical Director and Chief Medical Officer for psychiatric and medical crises.

• Document clinical interactions and outcomes thoroughly in patients’ records, adhering to our high standards of quality and compliance.

• Participate in our Quality Improvement Program, contributing to the development and implementation of best practices within the health center.

• Utilize evidence-based screening tools to assess the healthcare needs of individuals, providing critical data to inform treatment plans.

• Work alongside our Center of Excellence Team to support patients with Opioid Use Disorder, employing a compassionate and informed approach.

Patient Population / Client Focus\n

You will work closely with vulnerable groups seeking mental health support as part of an integrated care plan. Qualifications\n

To be successful in this role, candidates must possess the following qualifications:

• A Master’s degree in Social Work is required, along with an active and unrestricted LCSW license.

• A minimum of 2-3 years of clinical experience in social work, particularly in a healthcare setting such as primary or specialty care.

• Demonstrated ability to work collaboratively with a multidisciplinary team of health care providers, social service teams, and the patient community.

• This role may require travel to various CHDC locations for meetings, off-site retreats, conferences, and outreach programs.

Preferred Experience\n

Candidates with experience working in integrated behavioral health settings or those familiar with chronic disease management and preventative health will be highly regarded. Preference will be given to applicants who have experience with tools and methodologies that promote collaborative care.

Compensation and Benefits\n

We offer a competitive salary ranging from USD 90,000 to 100,000 per year, commensurate with experience.
