Licensed Clinical Social Worker (LCSW) - Remote Role in Hospice Care

Mind Full Counseling Center is actively seeking a dedicated and compassionate Licensed Clinical Social Worker (LCSW) for a full-time remote position. Work Environment

Key Responsibilities

• Conduct thorough evaluations and assessments of patients to identify their psychosocial needs and develop tailored care plans.
• Participate in the creation and implementation of Individualized Family Service Plans (IFSPs), ensuring that they align with both state and federal guidelines.
• Provide one-on-one counseling and support to patients and their families, assisting them in navigating their emotions and adjusting to end-of-life care.
• Work collaboratively with a team of healthcare professionals to monitor patient progress and modify care plans as necessary.
• Educate families about available resources, coping strategies, and evidence-based interventions that support holistic care.
• Lead support groups that empower families to express and process their feelings during challenging times.
• Maintain comprehensive documentation in compliance with all healthcare regulations and standards to ensure quality of care and continuity.
• Foster strong therapeutic relationships with patients and families, creating a supportive space for open communication and trust.

Patient Population / Client Focus

The LCSW will primarily focus on patients in hospice care, including those with terminal illnesses and their families. This role emphasizes culturally sensitive practices to support diverse backgrounds and unique family dynamics. You will work closely with families to address grief, loss, and the psychosocial impact of terminal diagnoses.

Qualifications

• A Master’s Degree in Social Work or Doctorate of Social Work from an accredited institution.
• Current licensure as a Licensed Clinical Social Worker (LCSW) by the Department of Public Health.
• A valid driver’s license to facilitate transportation, if needed.
• Exceptional communication and interpersonal skills to effectively engage with patients, families, and team members.
• Strong organization and time management skills.

Preferred Experience

• At least one year of professional experience working in hospice care or a similar field.
• Demonstrated competency in Natural Learning Environment Practices.

Compensation and Benefits
