Work Environment

This full-time position promotes a supportive and dynamic work setting, as you will be working onsite in patients' homes throughout San Francisco and the surrounding areas. Our focus on home care allows you to build meaningful relationships with your patients and directly observe the impact of your interventions in their daily lives. The work hours are Monday to Friday, from 9 am to 5 pm, enabling a balanced work-life schedule.

Key Responsibilities

• Conduct comprehensive evaluations of patients to establish their goals and individual needs.
• Maintain a patient load of one individual per hour, allowing for thorough assessments and focused treatments.
• Develop and implement personalized care plans utilizing diverse treatment techniques including manual therapy, therapeutic exercises, and modalities.
• Focus on providing cost-effective and evidence-based treatments to enhance patients' mobility and functional capabilities.
• Foster patient motivation through encouragement and education regarding the importance of active participation in their rehabilitation.
• Collaborate with other healthcare professionals to ensure a multidisciplinary approach to patient care.
• Document all services provided and maintain accurate medical records using EMR software such as WebPT.
• Evaluate treatment outcomes regularly and make necessary modifications to care plans based on patient progress.
• Encourage a commitment to healthy lifestyle choices focusing on strength, flexibility, balance, and coordination.
• Explore and recommend non-surgical alternatives to pain management and mobility enhancement.

Patient Population / Client Focus

You will primarily work with adult patients recovering from orthopedic conditions, including post-surgical rehabilitation, sports injuries, and chronic pain disorders. Our philosophy emphasizes tailored treatment plans that address the unique physical and emotional needs of each patient, ensuring that they receive compassionate care that promotes recovery in a familiar and comfortable environment.

Qualifications

• A Doctor of Physical Therapy (DPT) degree from an accredited program is required.
• Licensure as a Physical Therapist in California, CA, with no disciplinary actions against the license.
• Demonstrated proficiency in manual therapy techniques and developing effective exercise regimens.
• Experience using EMR systems, particularly WebPT, for documentation and patient management.
• Strong interpersonal and communication skills to establish rapport with patients and their families.

Preferred Experience

• A minimum of three years of experience as a Physical Therapist in an orthopedic outpatient setting is preferred.
• Prior experience in home care environments is an advantage.
• Skills in implementing innovative rehabilitation techniques and adapting treatment plans based on real-time feedback.

Compensation and Benefits

This position offers a competitive salary ranging from $100,000 to $125,000 per year, commensurate with experience.
