Work Environment

This position is onsite, requiring therapists to conduct therapy sessions in the homes of patients throughout the Chantilly area. Key Responsibilities

• Conduct comprehensive evaluations of patients, including reviewing their medical history, performing relevant assessments, and documenting findings to inform treatment planning.
• Develop and explain tailored treatment plans to patients and their families, ensuring their involvement in goal-setting to boost motivation and adherence.
• Implement targeted therapeutic protocols and treatments based on evaluation results and evolving healthcare practices, modifying approaches as needed for optimal patient outcomes.
• Collaborate with interdisciplinary healthcare teams to address patient needs effectively, recognizing changes in patient status that may require alterations in the treatment plan.
• Maintain thorough and accurate documentation of patient care activities, establishing a clear record of treatment progress, discharge planning, and follow-up procedures.

Qualifications

• A Bachelor's degree in Physical Therapy from an accredited institution is required.
• Licensure as a Physical Therapist in the state of Virginia is mandatory.

Compensation and Benefits
