Work Environment

This position is primarily onsite, specifically catering to home care settings. As a Physical Therapist, you will travel to patients' homes in the Manassas area, bringing essential care directly to them. Key Responsibilities

In your role as a Physical Therapist, you will be responsible for:

• Conducting comprehensive evaluations by reviewing medical histories, performing specific assessments, and documenting findings systematically to inform tailored treatment plans.
• Clearly articulating evaluation results and proposed treatment plans to patients and their families, while collaborating with them to establish achievable goals.
• Implementing individualized treatment protocols based on evaluation outcomes, ensuring the approach meets the evolving needs of patients and the health care landscape.
• Consulting with interdisciplinary healthcare professionals as necessary for holistic patient care, monitoring and documenting changes in the patient's condition, and modifying care plans accordingly.
• Handling discharge planning processes and ensuring effective follow-up care, while maintaining comprehensive documentation of patient interactions and treatment progress.
• Exhibiting exemplary time management skills to meet productivity targets and staying punctual for scheduled appointments and meetings.
• Contributing proactively to department goals and compliance with local and national healthcare regulations.
• Taking the lead in pursuing continuing education opportunities for your professional development and serving as a mentor for students.

Patient Population / Client Focus

Qualifications

Preferred Experience
