Key Responsibilities\n

In this role, you will:

• Conduct comprehensive assessments of students' psychosocial needs to develop individualized treatment plans that encompass short-term counseling, crisis intervention, and case management.

• Provide therapeutic support through virtual sessions, addressing a range of emotional and behavioral challenges.

• Assist students and their families in navigating and completing necessary paperwork for entitlements, supplemental insurance, and various community benefits.

• Act as a liaison and advocate for students, effectively communicating with community agencies and educational staff to ensure comprehensive care.

• Coordinate resources and transportation assistance for students needing support in accessing services.
\n
• Maintain thorough and accurate documentation of patient interactions, treatment plans, and progress records in our electronic health record (EHR) system.

• Participate in team meetings, case conferences, and quality improvement initiatives to continuously enhance service delivery.

• Conduct educational workshops for students' families, focusing on strategies for prenatal and postpartum wellness.
\n
• Monitor and follow up with students regarding missed appointments to ensure continuity of care.

Patient Population / Client Focus\n

Your primary focus will be on school-aged children and adolescents, addressing a diverse range of mental health issues including anxiety, depression, trauma, and behavioral disorders. You will play an instrumental role in empowering students to overcome challenges while fostering their resilience and emotional well-being.

Qualifications\n

We are looking for candidates who possess the following qualifications:

• A Bachelor's Degree in Social Work or a closely related field is essential.

• Applicants holding a Master’s Degree in Social Work (MSW) are preferred, with current Licensed Clinical Social Worker (LCSW) certification.

• Minimum of two years experience as a Social Worker is required, with five years preferred.

• Demonstrated proficiency in written and verbal communication, coupled with strong interpersonal skills.

• Familiarity with Microsoft Office Suite, including Word, Excel, and Outlook.
