Key Responsibilities\n

• Conduct therapeutic counseling, therapy, and psychotherapy as per clients' needs, while developing comprehensive treatment and discharge plans.

• Utilize various methods, including interviews and observational techniques, to gather and evaluate patient data effectively.

• Identify issues impeding patients' progress and provide appropriate therapeutic interventions or referrals to other specialists as necessary.

• Maintain an active caseload, managing individual therapy sessions, as well as collateral therapy for families and caregivers.

• Ensure meticulous review of client case files to guarantee accuracy, timeliness, and quality of documentation, including treatment plans, progress notes, and discharge summaries.

• Adhere to confidentiality regulations and ethical standards to maintain the trust and safety of clients throughout the therapeutic process.

• Participate in ongoing training to refine assessment protocols in accordance with best practices and health care regulations.

• Collaborate with interdisciplinary teams, providing insights into patients' psychosocial needs to enhance integrated care pathways.

Your work will focus on diverse patient populations facing significant health challenges, particularly those engaged with hospice care. Qualifications\n

• A Master’s Degree in Social Work from an accredited institution is required.

• An active and unrestricted Clinical Social Worker License (LCSW) is mandatory for this position.

Compensation and Benefits\n

The salary for this position ranges from USD 90,000 to 100,000 annually, commensurate with experience and qualifications.
